FBI Applicant Fingerprint Card Form FD-258

There are many agencies that require applicants to use the FBI Applicant Fingerprint Card Form FD-258 in order to conduct a FBI Identification Record—often referred to as criminal history record and/or FBI criminal background checks; also know as a “rap sheet”. Basically it is a listing of certain information taken from fingerprint submissions retained by the FBI in connection with arrests and, in some instances, federal employment, naturalization, or military service. All arrest data included in a FBI Criminal Background check is obtained from fingerprint submissions, disposition reports, and other information submitted by agencies having criminal justice responsibilities.

All data entered on ink fingerprint cards must be typewritten or legibly printed, preferably utilizing black over blue ink and must not exceed the boundaries of the designated field (block).

No. Due to concerns related to the protection of personally identifiable information, the FBI Applicant Fingerprint Card Form FD-258 will no longer be returned either for a “no record” response or with a FBI Identification Record.
